Welcome to SBCS School Health Clinic! I thought it might be helpful to
explain the programs and procedures of the health clinic. I am in the
clinic every day from 8:30a.m.~ 3:45p.m..
I treat the
students daily for minor illnesses and injuries. I can offer comfort
measures like ice packs, lotions for dry skin and lips, salt water
gargles for sore, dry throats, band aids for minor cuts and scrapes. I
call parents whenever the child has a fever over 100.0, vomiting,
significant pain, repeated complaints, or anything out of the ordinary.
If I am unable to reach a parent or if the situation does not warrant a
phone call but I feel it should be brought to your attention, I will
send home a note describing my concern and what you should watch for.
If your child has any serious allergies (food or dye allergies/ insect
stings) or chronic conditions (asthma, diabetes or seizures) please
alert their teacher and myself. Also be sure to keep the phone numbers
where you can be contacted during the school day current and notify us
of any changes.
For your
convenience I have outlined several of the policies of the health
room. Please take the time to review them, as your child may be
affected.

If you are in doubt as to whether
or not to send your child to school, please read
"Too
Sick for School"
for some general guidelines. I would also like to refer you to page 38 of
the St. John the Baptist Student/Parent Handbook regarding the
Accident/Illness
Policy. You will note that children will be sent home for any fever
over 100.0 degrees or contagious illness (including vomiting). In addition,
your child needs to be fever/symptom free for 24 hours after an illness
before returning to school. This
means that if your child is sent home from school, they should not return the
very next day, rather please allow for the 24 hour period from the time of
initial illness at which point they may return if they are symptom free.
Occasionally, I may send home a Medical/Dental Referral letter that
indicates your child needs to be examined by a physician.
Please attempt to have your child seen in a timely manner and return the
form to school.

SCHOOL
HEALTH PROGRAM
The
school nurse and the school health program supplements the efforts of
parent/guardian and health care providers to establish and maintain the welfare
and health of students. The program is not therapeutic and it does not replace
the parent/guardian responsibility to obtain health care.
The
nurse's office is located on the bottom floor of the building, next to the 5th
grade classrooms.
The responsibilities of the nurse consist of:

Providing emergency care (obtaining the necessary equipment for emergency care)

Keeping aware of changes in the health status of children so records can
be up‑dated.

Interpreting and checking each child's immunization record and then
informing parents of necessary required information missing from the health
record.

Assisting in the prevention and control of communicable disease.
-
Screening of vision, hearing,
height, weight and scoliosis for designated classes.
-
Sending reports to Hamilton
County and the State Department of Health as required.
-
Acting as a resource
person for students and school personnel and parents/guardians regarding
health services and information available in the community

GUIDELINES
FOR ADMINISTRATION OF MEDICATION BY SCHOOL PERSONNEL
This
information can also be found in the St. John the Baptist
Student/Parent
Handbook (pg 36)
When
it is necessary for school personnel to administer prescribed medication, the
following guidelines are to be followed:

All school personnel must be informed that the administration of any drug
(prescription or over-the-counter) without the order of the physician and
permission of the parent/guardian could be interpreted as practicing medicine
and is prohibited by law. |

2. |
The principal shall appoint a responsible person or persons to supervise
the storing and administering of the medication in the absence of the school
nurse.

3. |
Written request must be obtained from the physician and the
parent/guardian before ANY medication
may be administered by school personnel. The
request, completed on an
Administration of Medication at School
Form,
must be completed in its entirety and include instruction as to name of the
medication, dosage, time, reason for use, and duration of medication, and possible side
effects.
NO MEDICATION WILL BE GIVEN UNLESS ACCOMPANIED BY THIS FORM!

Medication
must be in original containers
(child proof) and have an affixed label including the student's name, name of
medication, dosage, route of administration, and time of administration. |

New
Administration of Medication at School request forms must be submitted each
school year and as necessary for
changes in the medication order.

It is advised that the medication (especially controlled substances) and
the signed permission forms be brought to school by the parent/guardian. School personnel cannot accept responsibility for medications
brought to school by students. This helps to prevent accidental
ingestion/lost medications
|
| 7. |
The school nurse is responsible for the monitoring of medications
administered by school personnel. The school nurse is responsible for providing
education including specific instructions pertinent to the medication.

Accurate records of the medication given must be kept in the student's
record |
| 9. |
If
a child carries an inhaler (for asthma), the school must be notified in writing
by the parent/guardian accompanied by a doctor's note as to the medical
condition and administration of the inhaler.
The doctor’s note must state that “the child may carry inhaler on
his/her person.”

HEALTH
SCREENINGS
A program of early identification,
diagnosis and correction of childhood vision and/or hearing disorders in an
essential part of all child health programs. The early detection and
treatment of vision disorders give children a better opportunity to develop
educationally, socially, emotionally and physically.
Vision and Hearing screening
procedures required by the
Ohio Department of
Health (ODH) are done under the authority of the Ohio Revised Code (O.R.C.),
section 3313.69.
Students in grades
K,1,3,5,and 7 will have their
vision screened and students in grades K,1,3, and 5 will have their hearing
screened, this year during the fall. If a student fails
a screening twice, a referral will be sent home. A physician’s report is
included with each referral and this should be completed by the physician and
returned to school so the information can be entered in the student's health
record. Please complete the forms as soon as possible after receiving them
or contact me if there is a problem getting them completed. I will be
happy to work with you.
A
Postural Screening Program will be conducted every January for grades 6~8.
The screening will help identify children with curvature of the spine
called scoliosis or other possible spinal problems.
Scoliosis affects one in every twenty persons.
Two or three out of every hundred diagnosed with a spinal problem will
require active treatment. The
purpose of this program is to recognize the problem at its earliest stages so
that the need for treatment can be determined.
The
procedure for screening is a simple one in which a trained examiner inspects the
child’s' posture as he or she stands and then bends forward.
If
a postural problem is suspected, a second screening will be conducted by a nurse
from the Greater Cincinnati Spinal Deformities Center.
If a follow-up by your physician is indicated, you will receive a letter
after the second screening
If
you do not wish for your child(ren) to participated in any of the above
screening procedures, please send a letter to the school nurse prior to the
screening dates.
